By clicking "Next", I hereby consent to allow BERENGUER & ASSOCIATES (hereafter referred to as COMPANY), through its designated agent/employee, to obtain and verify my consumer reports, including but not limited to, my credit report, criminal information, and eviction information for the purpose of determining my eligibility to lease an apartment. I further understand if I lease an apartment, I consent to allow COMPANY and its designated agent/employee, for the duration of my lease, to review the following list of information to assess risk, for analytics, for process improvement, and other uses: my consumer reports, including but not limited to my credit report, criminal information, eviction information, my rental payment history, and occupancy history, and other information. The facts set forth in my application for residency are true and complete. False, fraudulent or misleading information on an application may be grounds for denial of residency or subsequent eviction.

This authorization is to remain in full force and effect until the COMPANY has received written notification from me (or either of us) of its termination in such time and in such manner as to afford the COMPANY a reasonable opportunity to act.

You will be charged an application fee of $99.00, and an online access fee of $19.00.

Terms and Conditions

The card above will be charged $99.00 as an application fee. The card above will also be charged $19.00 for the services of Property Matrix online applications. I agree to the terms that these charges are NON-REFUNDABLE.

I(We) do hereby authorize BERENGUER & ASSOCIATES hereafter named COMPANY, to initiate to (my/our) account indicated at the depository financial institution named above, hereafter named FINANCIAL INSTITUTION. I further authorize COMPANY to initiate an adjusting or correcting entry as necessary. Finally, should any such debit(s) be returned as NSF or Uncollected Funds, I(we) authorize the COMPANY to collect such debit(s) electronically and to subsequently collect a Returned Item Fee of $25.00 (or the maximum allowed by state law, whichever is greater) per item, electronically from the same account identified above.

I am a duly authorized signer on the account identified above, and authorize all of the above as evidenced by my clicking of the agreement button and submission of this form.

Property Matrix is an independent property management software company. By using this service, you are contracting with Property Matrix to process this transaction. The Online Access Fee is an amount charged by Property Matrix to you the customer for payment access through the Property Matrix online applications. The Online Access Fee is nonrefundable.

I Authorize the Online Access Fee to be charged to the card provided above. Online Access Fee will appear on your statement as PM* BERENGUER & ASSOC.
